Meet Dr. Jaxon Kramer

Dr. Jaxon Kramer leads The Welcome Home Practice with over a decade of NHS clinical and leadership, research, and policy work. His specialist expertise spans across the lifespan to include child, adolescent and adult mental health, neurodevelopmental conditions (with a focus on tic disorders), trauma, and complex emotional needs.

Grounded in lived neurodiversity insight and a deep commitment to compassionate, evidence-based care, Dr. Kramer provides personalised therapy, detailed assessments, parent and family consultation, and strategic consultancy services for individuals, families, and organisations.

At The Welcome Home Practice, we believe strongly in person-centred care. Every client is met with respect, empathy, and tailored support designed to meet their unique needs across the lifespan. This approach ensures a safe space where clients and referrers alike feel understood and empowered.

“The Welcome Home Practice is your safe therapeutic home — a place to be seen, heard, and supported on your journey to growth and wellbeing.”

Meet Leanne Makarem

Leanne is our Senior Assistant Clinical Psychologist at The Welcome Home Practice, where she plays a key role in supporting the wider psychological team and contributing to the smooth, thoughtful delivery of care across the service. She works closely alongside clinicians and clients to help ensure that assessments and therapeutic work are held and delivered within a compassionate, reflective, and well organised framework.

Leanne brings a careful and considered approach to her role, with a strong commitment to the values that sit at the heart of the practice. She is particularly mindful of the importance of understanding each child or young person within their wider context, and of supporting work that is respectful, person centred, and sensitive to neurodiversity.

Known for her calm presence, her attention to detail, and a quietly reassuring sense of humour, Leanne helps to create an environment that feels both contained and human. Her work reflects the practice’s emphasis on warmth, authenticity, and care, supporting clinicians and families alike to feel held and thoughtfully supported throughout their time with the service.
